This is the single highest-rejection area in hazardous-vehicle claims. Verify ALL six — the DL class, hazardous endorsement, PESO certificate, refresher receipt within 12 months (critical — a 13-month-old refresher is often enough for an insurer to deny), transport badge, and age / experience. For PESO training: the trainer must be a PESO-approved institute; obtain the certificate serial number so insurer can cross-check.
